How to Make a Referral
Rising Above Disabilities accepts referrals from Service & Support Administrators, case managers, families, and community partners.
To refer an individual for services:
• Contact our team by phone or email
• Provide basic information about the individual’s service needs
• Our team will coordinate with the Service & Support Administrator to determine service availability and next steps
